ABSTRACT

Objective:To observe the ferroptosis in hippocampal neurons of rats with multiple cerebral concussion (MCC) .Methods:Ninety clean grade male Wistar rats with a body mass of (250±10) g were randomly divided into control group (12 rats) and model group (78 rats) according to the random number table method. The rat of MCC model was prepared by hitting the frontotemporal lobe of rats with free fall method once a day for 3 consecutive days.Then the MCC model rats were randomly divided into 12 h, 24 h, 48 h, 72 h, 7 d and 14 d groups with 12 rats in each group. The balance beam experiment was used to detect the motor coordination function of the rats. The levels of interleukin-β(IL-1β), interleukin-6(IL-6), serum glutathione (GSH), neuron-specific enolase (NSE) in serum of rats were detected by ELISA.The content of iron ion in hippocampus was detected by colorimetry. The mRNA and protein levels of glutathione peroxidase 4(GPX4), ferritin heavy (FTH) and ferritin light(FTL) in the hippocampus were detected by RT-PCR and Western blot. Prussian blue staining was used to observe the iron deposition in brain tissue.The ultrastructural changes of hippocampal neurons and mitochondria were observed by transmission electron microscope. SPSS 24.0 statistical software was used for one-way ANOVA among groups, and LSD test was used for multiple pairwise comparison.Results:In the balance beam experiment, the passing time and motor coordination score of rats in each group were significantly different ( F=30.08, 60.34, both P<0.05). The passing time and motor coordination score of rats in the 48 h group ((87.00±4.74) s, (4.75±0.43)) were significantly higher than those in the control group ((35.13±6.99) s, (0.75±0.23)) (both P<0.05). There was significant difference in the total iron ion content, Fe 2+ content and Fe 3+ content in hippocampus of rats in each group ( F=25.20, 94.42, 40.25, all P<0.05), and the content of Fe 2+ in hippocampus of 48 h group was significantly higher than that of the control group ((10.17±0.05) ng/μL, (8.65±0.01) ng/μL)( P<0.05). In the results of RT-PCR and Western blot, the mRNA and protein levels of GPX4, FTH and FTL in hippocampus of each group were significantly different ( F=37.94, 82.09, 49.01, 71.63, 28.94, 15.78, all P<0.05). The mRNA level and protein level of GPX4 ((1.09±0.01), (0.23±0.01) )and FTL ((1.60±0.03), (0.64±0.02)) in 24 h group were significantly higher than those of the control group (GPX4: (1.00±0.02), (0.17±0.01)), FTL: ((1.00±0.04), (0.32±0.01))(all P<0.05). The mRNA level and protein level of FTH ((0.24±0.03), (0.07±0.01)) in 24 h group were significantly lower than those of the control group((1.00±0.01), (0.67±0.03))(both P<0.05). The results of electron microscope showed that the hippocampal neuronal cells of the model rats were reduced, the nucleolus was broken and the nuclear membrane was shrunk in varying degrees, the mitochondria were swollen and deformed and there were vacuoles, and the cristae in the mitochondria decreased or disappeared. Conclusion:The levels of inflammatory reaction and oxidative stress in the multiple cerebral concussion model rats increase, and the hippocampal neurons show the characteristics of ferroptosis, especially at 24 h and 48 h.

Abstract Background: Brain concussion (BC) is seen as a public health priority due to its high incidence and morbidity rate, among thousands of people around the world. There are needs for fast identification, accurate diagnosis and correct management in order to reduce the short and long-term problems relating to BC. Proper knowledge of BC in the population and among clinicians is a critical factor in achieving this. Objectives: To evaluate the level of self-reported BC knowledge and gaps/misconceptions, and to identify variables correlated with this level. Methods: A cross-sectional descriptive survey was performed. A Brain Concussion Knowledge Questionnaire (BCKQ) that had been created to capture data was widely distributed. Total scores, domain partial scores and percentages of correct and incorrect answers were calculated to ascertain the level of knowledge relating to BC. Results: The sample was formed by 1,247 Brazilian adults (age: 41.7±11.8 years). Partial scores of the BCKQ revealed the existence of poor knowledge and misconceptions in all domains of the questionnaire, especially regarding questions about recovery from and management of BC. Moderate correlations between BCKQ scores and professions (p=0.312; P=0.00) or previous brain concussion knowledge (p=0.489; P=0.00) were observed. In a multiple linear regression model, age, profession and sports practice were predictors of BC knowledge. Conclusion: This first study to analyze the level of BC knowledge in a sample of Brazilian adults suggests that poor knowledge and misconceptions are present. Thus, meaningful and useful information was provided by this study for developing health education programs about BC for the population in order to improve fast diagnosis and correct BC management.


RESUMO Introdução: A concussão cerebral (CC) é considerada prioridade em saúde pública devido à sua alta incidência e taxa de morbidade. Há necessidade de identificação rápida, diagnóstico acurado e manejo correto após essas lesões, com objetivo de reduzir problemas de curto e longo prazo. O conhecimento apropriado da população, atletas e de clínicos é primordial para que isto seja alcançado. Objetivos: Avaliar o nível de conhecimento autorrelatado sobre CC, descrever lacunas/equívocos e identificar variáveis correlacionadas com este nível. Métodos: Foi realizada uma pesquisa descritiva transversal. O Questionário de Conhecimento sobre Concussão Cerebral (QCCC) criado foi amplamente distribuído. Pontuações totais e parciais em domínios e porcentagens de respostas corretas e incorretas foram calculadas para obter o nível de conhecimento sobre CC. Resultados: A amostra foi formada por 1.247 brasileiros adultos (41,7±11,8 anos). Pontuações parciais do QCCC revelaram baixo nível de conhecimento, principalmente em questões sobre recuperação e manejo. Observou-se correlações moderadas entre a pontuação no QCCC e a profissão dos participantes (p=0,312, P=0,00) ou conhecimento prévio sobre CC (p=0,489, P=0,00). A regressão linear múltipla mostrou que a idade, profissão e prática de esportes podem ser preditoras do conhecimento sobre CC. Conclusão: Esse primeiro estudo a analisar o nível de conhecimento sobre CC em uma amostra de adultos brasileiros sugere baixo conhecimento e equívocos sobre esta condição. Assim, informações úteis e significantes foram fornecidas para o desenvolvimento de programas de educação em saúde sobre CC para o público, com objetivo de melhorar a identificação rápida e o manejo correto de concussões.

ABSTRACT

Objective To detect the long term effect of pure and multiple concussions on spatial cognitive of rats.Methods One hundred and eighty 7-week-old Spragne-Dawley male rats with weight of 280 ± 30g were chosen and randomly divided into a control group and a concussion group.The cerebral concussion was induced in the rats using a metallic pendulum striker concussive device.After the first strike,the brain injury group was randomly divided into a pure cerebral concussion(PCC)group and a multiple cerebral concussion(MCC) group.After the second strike,the MCC group was randomly divided into two-fold cerebral concussion(2MCC) group and three-fold cerebral concussion(3MCC) group.The striking interval was 24h.One,3 and 6 months after trauma,their cognitive function was tested using Morris water maze.Results One month later after injury,there was no significant difference in the escape latency between the control group and PCC group.Significant differences in the measurement were observed between the control/PCC group and 2MCC group on the 7th day after the injury,also between the control/PCC and 3MCC groups on the 6th and 7th day.And there were significant differences between the 2MCC and 3MCC groups on the 6th and 7th days.The non-platform test did not observe any significant differences among the four groups.Three months after injury,there was still no significant difference between the control group and PCC group,PCC and 2MCC groups,as well as 2MCC and 3MCC groups in the escape latency.However,there was significant difference between the control group and 2MCC group on the 5th,6th and 7th days,between the control group and 3MCC group on the 4th,5th,6th and 7th days,as well as between PCC group and 3MCC group on the 6th and 7th days.In the non-platform test,there was no significant difference between the control group and PCC group,between PCC group and 2MCC group,as well as between 2MCC group and 3MCC group.However,2MCC and 3MCC groups spent significantly less time in the former platform quadrant,when compared with the control group and 3MCC group spent significantly less time than PCC group.Six months after injury,significant differences in the escape latency were observed between the control group and PCC group on the 6th and 7th days,and 2MCC group on the 5th,6th and 7th days,also and 3MCC groups on the 2nd,3rd,4th,5th,6th and 7th days,still between PCC group and 2MCC group on the 6th and 7th days,as well as between PCC group and 3MCC group on the 4th,5th,6th and 7th days.Moreover,there was significant difference between 2MCC and 3MCC groups only on the 7th day.In the non-platform test,PCC group,2MCC group and 3MCC group spent significantly less time in the former platform quadrant compared with the control group.Moreover,in this test significant differences were found between PCC group and 2MCC/3MCC group,but not between 2MCC group and 3MCC group.Conclusion With the increase of cerebral concussion times,earlier and more serious damage of spatial cognition will appear,with a significant cumulative effect in rats.Such rat model can be used to study the pathological changes of cognitive impairment in chronic traumatic encephalopathy.

ABSTRACT

Central pain, a neuropathic pain caused by an injury or dysfunction of the central nervous system, is a common, annoying sequela of mild traumatic brain injury (mTBI). Clarification of the pathogenetic mechanism of central pain is mandatory for precise diagnosis, proper management, and prognosis prediction. The introduction of diffusion tensor imaging allowed assessment of the association of the central pain and injury of the spinothalamic tract (STT), and traumatic axonal injury (TAI) in mTBI. In this review, 6 diffusion tensor tractography studies on central pain due to TAI of the STT in patients with mTBI are reviewed. The diagnostic approach for TAI of the STT in individual patients with mTBI is discussed, centering around the methods that these studies employed to demonstrate TAI of the STT.

ABSTRACT

Objetivo: presentar la reducción progresiva en la frecuencia de conmociones cerebrales en los Campeonatos Mundiales de Karate, a partir de los cambios en el reglamento de la Federación Mundial de Karate (FMK). Material y Método: Estudio prospectivo de las conmociones cerebrales ocurridas en los campeonatos del mundo de Madrid (2002), Monterrey (2004), Tampere (2006), Tokio (2008), Belgrado (2010) y París (2012). Resultados: La frecuencia de conmociones cerebrales, definidas según la Conferencia Internacional de Conmoción en el Deporte (Viena 2001, Praga 2004, Zurich 2008 y 2012) ha disminuido progresivamente en los Campeonatos del Mundo de Karate: una conmoción cada 230 combates en Madrid, una cada 196 en Monterrey, una cada 99 en Tampere, una cada 612 en Tokio, una cada 512 en Belgrado, y una cada 1.140 en París. Discusión y Conclusión: El uso de protecciones, la aplicación rigurosa del reglamento del karate deportivo y la educación continua en la prevención de lesiones de los competidores, técnicos y árbitros, ha disminuido la frecuencia de conmocionescerebrales en los Campeonatos del Mundo, haciendo del karate deportivo una disciplina más segura, cumpliendo con los principios del Olimpismo.


Objective: to present the progressive reduction of the frequency of cerebral concussion in the World Karate Championships, due to the changes in the Rules and Regulations of the World Karate Federation (WKF). Material and Method: Prospective study of the brain concussions occurred in the World Championships of Madrid (2002), Monterrey (2004), Tampere (2006), Tokyo (2008), Belgrade (2010) and Paris (2012). Results: The frequency of brain concussion, defined by the International Conference of Sports Concussion (Vienne 2001, Prague 2004, Zurich 2008 and 2012) has progressively diminished in the World Karate Championships: one concussion each 230 bouts in Madrid, one each 196 in Monterrey, one each 99 in Tampere, one each 612 in Tokyo, one each 512 in Belgrade, and one each 1,140 in Paris. Discussion and Conclusion: The use of protections, the rigorous enforcement of the Rules and Regulations of the Sports Karate and the continuous education inthe prevention of injury of athletes, technicians and referees, has diminished the frequency of brain concussion in the World Championships, making Karate a much safer sport, achieving Olympic principles.

ABSTRACT

Objective To test the reliability and validity of Chinese version of Stressful Life Events Questionnaire and the influence of preinjury stressful life events on the life quality and mood symptoms of the patents following cerebral concussion.Methods 285 patients with cerebral concussion were assessed by Stressful Life Events Questionnaire(SLESQ),PTSD Checklist civilian version(PCL-C),Thirty-Six-Item Short-Form Health Survey (SF-36),Hamilton Depression Rating Scale(HDRS),Hamilton Anxiety Scale(HAMA) at baseline.The patients had been reassessed with the HDRS,HAMA,SF-36 at 3 months after cerebral concussion.The hierarchical regression analyses were used to determine the influence of Stressful Life Events for the life quality and mood symptoms of the patient with cerebral concussion.Results The incidence of stressful life events was 42.8%.The multiple regression analysis showed the experience of stressful life events were the influence factor of scores on the SF-36,HDRS,HAMA after adjusted by the age,gender,education,income,social support,the scores of anxiety and depression before injury and the R 2 value of SLESQ to SF-36 physical health factor,mental health factor,HDRS,HAMA was 0.064,0.111,0.145,0.028,respectively.Conclusion The history of stressful events is the influence factor of the patient with cerebral concussion to have poor life quality and mood disorder.

ABSTRACT

ObjectiveTo observe the prepulse inhibition(PPI) ot the startle reflex of pure cerebral concussion (PCC) suffered from one concussion and multiple cerebral concussion (MCC) suffered from three concussions in rats,and to explore accumulate effect upon cognitive dysfunction of MCC.MethodsA metallic pendulum striker device for closed head injury was employed to duplicate PCC and MCC models in Stragu-Dawley rats.The MCC rats were hit three times on rats'head and it is interval 24 hours for every hit.According to the criteria of cerebral concussion,the investigated animals were divided into PCC group and MCC group at freedom.One control group was used.Each group included 10 animals.Each experience mental animal was tested from 3 days pre-injury to 28 days post-concussion.Startle reflex amplitude (for P values),pre-stimulation induced reflex amplitude on three standard stimulations,that was,67dB,69dB and 73dB (for PP67,PP69 and PP73 values) and prepulse inhibition of the startle reflex (PPI) were collected.ResultsThe P values and three PP values in the first three days of pre-replication experiment,there was no statistical significance in each group.However the P values and PP67,PP69 and PP73 values declined until to the 16th day after injury (P<0.05),then recovered in PCC group.The P value and PP67,PP69 and PP73 values changes of MCC group declined and not recovered until to test end (P<0.05 ) and they were more lower than PCC.The three PPI values were a little bit increase in both groups,there were statistics significance at some test points (P<0.05) compared with control.ConclusionThe startle reflex amplitude and pre-stimulation induced reflex amplitude weaken after cerebral concussion and there is damaging accumulate effect to injury times,the PPI is enlanced by cerebral concussion.

ABSTRACT

Behcet disease (BD) is rare in childhood. We report a 9-year-old boy with neuro-Behcet disease who presented diplopia and weakness on the left side after a cerebral concussion. Brain magnetic resonance imaging (MRI) revealed hyperintensity of the right mesodiencephalic junction on T2-weighted and fluid attenuated inversion recovery images. Prednisolone administration resulted in complete remission and normalization of abnormal MRI finding. Brain MRI is a useful diagnostic tool when the neurological sign is the first symptom of subclinical BD.

ABSTRACT

Este trabajo revisa la definición de conmoción cerebral asociada al deporte y sus posibles complicaciones. Se muestra el perfil de lesiones del XX Campeonato Panamericano Juvenil de karate, especialmente la frecuencia de traumatismo encéfalo craneano (TEC). Finalmente propone un protocolo de evaluación precompetitiva, examen neurológico en el área de competición y normas para el reintegro a la actividad deportiva post TEC.


This work reviews the definition of sport concussion and it’s complications. The injury profile of the XX Panamerican Karate Junior Championship, especially frequency of head trauma is shown. Finally proposes a protocol of precompetitive evaluation, a neurological exam in the competition area and rules of eincorporation to sport activity after concussion.

ABSTRACT

Objective To observe learning and memory behavior changes of one time cerebral concussion called pure cerebral concussion(PCC)and three times cerebral concussion called multiple cerebral concussion (MCC)after injured 24 days in rats.Methods A metallic pendulum striker device was deployed to duplicate PCC and MCC model in SD rata which were the complete closed head injury model.The animals were divided into PCC and MCC groups at random.One control group was used,each group has eight animals(n=8).One 8-arms radial maze was used to assessed each animal's capabilities,that is,spatial reference memory,working memory,spirited activity and take in food.Results Compared with control group,there were some significance(P<0.05)in both experiment groups post injury,that was,(1)The food intake decreased,PCC group from the 1st to the 11th day(from 0.00±0.00 to 2.62±1.76)after injury,MCC group from the 1st day to the 24th day(from 0.00±0.00 to 0.75±1.48)after injury.(2)Spirited activity depressed,PCC group on the 1st to the 7th,13rd day(from 4.87±1.24 to 10.0±2.39)after injury,MCC group on the 1st to 8th,22nd day(from 4.25±5.03 to 9.37±4.20)after injury.(3)The spatial reference memory was lower in early then gradually increased,PCC group on the 1 st to 7th day(from 0.50±0.75 to 3.O0±1.06)after injury,MCC group from the 1st to 19th day(from 1.88±2.10 to 2.50±2.44)after injury.(4)The working memory was delaying damaged,PCC group from the 1st to the 6th day and the 10th to the 23rd day(from 0.00±0.00 to 4.25±3.05)after injury,MCC group on the 1~4th,6th,9~13th,15th,16th,19th~22nd day(from 0.25±0.46 to 3.12±2.87)after injury.Conclusion The injured rata'capability of spatial reference memory,working memory,spirited activity and food intake were obviously damaged after CC,and the MCC group's capability of spatial reference memory,spirited activity and food intake was worse than PCC group.

@#ObjectiveTo observe the manifestations of depression at different stages after cerebral concussion. Methods77 cerebral concussion patients received Hamilton Depression Rating Scale (HAMD) examination at acute stage (in the 7 days after disease) and convalescence stage (at about 30 days after disease), and then compared with 48 health adults. ResultsThe scores of anxietas somatization, cognitive handicap, retardation, dyssomnia, feeling of despair conclude the total score in the cerebral concussion patients at acute stage were all significantly higher than that of the control (P<0.05). During the convalescence stage, the scores of anxietas somatization, the change of body weight, cognitive handicap, diurnal variation, retardation, dyssomnia, feeling of despair conclude the total score were all significantly higher than that of the control (P<0.05). The scores of almost all the item of HAMD were significantly higher than that of the control at both acute stage and convalescence stage, and the scores of athymia, suicide, difficulty falling asleep, early awake, agitate, somatization, constitutional symptom, hypochondriasis, loss of weight, insight, diurnal variation, feeling of despair, anosognosia, sense of inferiority and total HAMD scores at convalescence stage were all significantly higher than that at the acute stage, but the scores of blockage, psychoanxietas, derealization, symptom of over-absorption mind, compel symptom, sense of capability decrease at the acute stage were all significantly higher than that at the convalescence stage (P<0.05).ConclusionThe manifestations of depression in the cerebral concussion patients were not completely the same, so the mental intervention should be in direction.

ABSTRACT

Objective To observe the change of brainstem auditory evoked potential(BAEP)of cerebral concussion patients.Methods The results of BAEP of 60 patients with cerebral concussion were analyzed and were compared with the results of CT test.Results Among the results of 60 cases of cerebral concussion,the abnormal rate of BAEP was 83%;the abnormal rate of CT test results was 21%.Conclusion BAEP is of great value in the early diagnosis of cerebral concussion and in the efficient evaluation of the function of brain stem.

ABSTRACT

Objective To study changes and significance of endothelin(ET) in rat cerebral concussion.Methods 80 Wistar male rats were used for animal model of cerebral concussion,which were sacrificed on 1,3,7,14 and 30 days after injury and the brain tissue were taken off. The expression of ET was studied in the course of cerebral concussion by means of immunohistochemistry.Results Typical clinical manifestation was observed in the 100 g group in which the pathological changes included cerebral vascular constriction and dilatation,congestion and edema of cerebral tissue,neuronal degeneration,necrosis,and obviously decreased even disappeared Nissl bodies.Increased expression of ET was observed on the first day,the positive area was seen in the plasma of endothelial cells in cerebral cortex,hippocampus,cerebellum and thalamus.ET expression peak occurred on the 7th day,the positive area was also found in the plasma of Purkinje cells in the cerebellum.Decreased ET expression was found on 14th day and returned to normal level on the 30th day.Conclusion The main pathological changes of cerebral concussion contained blood circulation disorder,and degeneration and necrosis of substantial cells.ET was involved in the brain tissue injury during the pathological process of cerebral concussion and might be related to regulation of cerebral vascular reaction,and neuron degeneration and necrosis.

ABSTRACT

Altogether 80 Wistar rats were used for an animal model of cerebral concussion, which were sacrificed on days 1,3,7,14 and 30 after injury and the brain tissue was collected. The pathologic changes of cerebral concussion and apoptosis of nervous cells were studied by means of light microscopy, electron microscopy and in situ terminal end labeling method. The results showed that the clinical situation for cerebral concussion occurred in rats struck by 100g standard weight from 1 meter high. The basic pathologic changes were the cerebral vascular dilatation, congestion, hemorrhage, and edema of cerebral tissue. Nervous cells underaent degeneration, apoptosis and necrosis, and the Nissl bodies obviously decreased, even disappeared. On days 1~3 after injury, dot or piece necrosis was seen in brain tissue, around which the tissue rarefied. Monocytes and foam cells increased, and lots of neurons underwent degeneration, apoptosis and necrosis. The edema of cerebral tissue reached its peak on day 7. Hippocampal pyramidal cells decreased in number and showed the changes of obvious ischemia. On days 14~30, blood vessels also showed dilatation, congestion and hemorrhage, whereas edema alleviated. The neurons in cerebral cortex and hippocampus also showed the changes of chronic ischemia. By in situ terminal end labeling the number of apoptotic neurons increased on day 1, reached its peak on day 3 and still existed on day 30. The results suggested that the main pathologic changes of cerebral concussion were blood circulatory disorder and nervous cell degeneration, apoptosis and necrosis. Apoptosis of nervous cells was one of the main changes in cerebral concussion.

ABSTRACT

An experimental model of cerebral concussion induced by blunt force stroke in 46 guinea pigs was established. 30 out of 46 animals were unconscious immediately after stroke followed by recovery Within few minutes without any seuqel in the posttraumatic period The animals were killed at 3,6,12,24,48 and 72 hours, 1,2,3,4 weeks after injury. Within 72 hours after injury, some of axons of both the afferent and the efferent nerve fibers of the corpus callosum and the brain stem were swelling like necklace or disrupted into segments macrophage infiltration, glosis, satellitosis, pyknosis of nuclioli of neuron, lysis of nissles bodies, vascular reaction as well as the retraction of the torn ends of axon as demonstrated by the silver stain were observed in animals killed 24 and 48 hours after injury. The reaction mentioned above became milder and milder 1~4 weeks after injury. Brain contusion were produced in other 16 experimental animals. In 7 animals of control group, no change was observed

This paper reports various cases of cerebral concussion which strongly supportthat cerebral concussion may be regarded as a graded phenomenon with practicalvalue for clinic forensic medicine.The diagnosis,neuropathological changes of the central nevous system,typing ofconcussion are discussed in this paper.The pathological changes of the brain hase been analysed.
